Patient Education: Hospital Discharge and Readmission
Patient education is defined as a process of assisting consumers of health care to learn how to incorporate health-related behaviors (i.e., knowledge, skills, and/or attitudes) into everyday life with the purpose of achieving the goal of optimal health. Patient education is not health information seeking, or the intentioned choice to consume information about health, illness, health promotion, or risks to health. Patient education focuses on interaction and engagement of patients or individuals who are directly interacting with health care providers and services for personal health.
