This chapter analyses the efforts of a project team in a high technology organization that set out to identify past mistakes in product design and manufacture in order to reform future practices. At the time when the project team was set up, the company was endeavouring to adjust to a range of changes in its methods of design and manufacture and its task was given high priority.
