This appendix contains philosophical statements, a nursing practice standard, assessment standards, and documentation forms originally developed by a community health nursing service (the Vancouver Health Department in Vancouver, Canada) that were used by nurses in the home care nursing program and the prevention program. Note the theoretical consistency between the standards, the statements, and the forms.
