Four nursing forms based on the problem-oriented approach to medical records are included in this appendix. The assessment form used for gathering data for the action plans and progress notes for the rehabilitation population is not included. The forms shown here summarize the assessment findings needed to determine the actions to be taken and indicate changes in self-care status that may occur. A variety of methods may be used to gather the information, such as physical inspection, interview, and consultation of other records. The forms shown present that information as conclusions drawn, judgments, and decisions made.
