The health care system in the United States is continuing to undergo sweeping changes. Both the private health (medical) delivery and the public health sectors are being challenged by structural changes and fiscal constraints. Examples of these changes include the formation of new corporate entities emphasizing integration of services, downsizing, managed care, and the privatization of some public health services. These factors are encouraging the development of new partnerships between public and private entities and causing a reexamination of the roles both sectors play in improving health in communities (Lasker & the Committee on Medicine and Public Health, 1997).
