In this chapter, we present an in-depth analysis of after-action reviews, the most frequently discussed type of organzational learning mechanism (OLM) in the literature on organizational learning. The literature on this OLM describes after-action reviews in numerous settings, including high-hazard plants (Carrol, 1995, 1998), Microsoft (Cusumano & Selby 1995), British Petroleum (Gulliver, 1987), and the U.S. Army (Baird et al., 1997). These descriptions focus on the procedural aspects of the after-action reviews and the stages that are followed.
