Medication Therapy Management (MTM)

Medication therapy management (MTM) is defined as a service that is provided to an individual patient to assess the appropriateness and completeness of medication therapy. The purpose of MTM services is to identify medication or medical concerns and offer recommendations to improve health outcomes. MTM services can be provided personally (face to face) or by phone. In general, pharmacists provide these services, but other health care professionals can also administer MTM services. The provided services differ greatly from traditional prescription dispensing and counseling done by the pharmacist, as it is more comprehensive and patient-centered.
