A release of information is a written document signed by a client or clients that gives permission for the therapist to share confidential information with someone outside of the therapeutic relationship. It may also be referred to as an authorization to release information. Individuals to whom information may be released include physicians, psychiatrists, school personnel, other mental health professionals, social service organizations, attorneys, court personnel, family or friends of the client, or anyone else with whom the client would like information about their treatment to be shared.
