Case Management and Care Coordination

Case management and care coordination are related supports that connect people with intellectual disabilities to appropriate services and supports to meet identified medical, social, employment, residential, or other identified goals. In general, case management for people with intellectual and developmental disabilities is associated with long-term supports and services and is provided under the auspices of government social service agencies that provide disability-related services. Care coordination, often called health care coordination, is usually associated with the health care system and is often used in the management of complex medical conditions.
