Health Care Transition Planning

Health care transition planning refers to the comprehensive process of service preparation and support involved in the transfer of care from pediatric health care to adult health care systems and to other adult service systems for youth with special health care needs and disabilities and their families. Health care transition planning involves the following components of an individualized transition plan of care to support youth with special health care needs and disabilities and their families to better prepare them for emerging adulthood and beyond: (a) transfer to adult specialty and primary care providers accompanied by a health summary or health passport; (b) enrollment in a health insurance plan once the individual's pediatric eligibility terminates; (c) acquisition of the skills and knowledge needed to manage the ...
