The SF-36® Health Survey is a 36-item questionnaire used to assess patient-reported health-related qualityof-life outcomes. The SF-36® Health Survey measures eight domains of health:

1. Physical functioning

2. Role-Physical

3. Bodily pain

4. General health

5. Vitality

6. Social functioning

7. Role-Emotional

8. Mental health

It yields an eight-scale profile of norm-based scores (one for each of the eight domains of health) as well as physical and mental health component summary scores, a self-reported health transition rating, a response consistency index (RCI), and a preferencebased health utility index (SF-6D).
