Few would argue that market forces in the United States, most notably managed care, have fundamentally changed the ways in which health care services are offered by providers, accessed by clients, and acquired by payors. Between 1988 and 1998, the proportion of persons insured in the United States enrolled in managed care—including health maintenance organizations (HMOs), point of service (POS) plans, and preferred provider organizations (PPOs)—increased from approximately 29% to 86% (Goldstein, 1999).
