Psychiatric diagnoses in the United States are made based on the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). This manual is used to diagnose individuals and interventions are designed based on the diagnosis. However, couple and family clinicians tend to believe that individual (intrapersonal) issues influence and are influenced by interpersonal relationships and problematic relational patterns. Although the DSM-5 is primarily used to diagnose individual issues, relational diagnoses (referred to as relational problems) appear in the DSM-5 as part of the category “other conditions that may be a focus of clinical attention.” These conditions are not considered mental disorders in the DSM but may affect the diagnosis, course, prognosis, or treatment of a patient’s disorder.
